Procedure for disclosure requests, etc.

We use the following procedure to respond to requests from an individual or their proxy for notification of the purposes of use, disclosure, correction, addition or removal, suspension of use, deletion, suspension of provision to third parties, or disclosure of records of provision to third parties (hereinafter, “disclosure requests, etc.”) regarding the retained personal data in our possession.

1. Submitting disclosure requests
   To request disclosure, etc., please append the necessary documents to the Personal Information Disclosure Request Form and submit it via email attachment, fax, or post. If submitting by post, please use a method that allows the delivery records to be checked, such as recorded delivery or registered mail. Please also mark the envelope “Contains personal information disclosure request forms” in red ink.
2. Paperwork to submit for disclosure requests, etc.
   To request disclosure, etc., please complete all of the specified fields on the personal information disclosure request form.
3. Identity verification
   We will verify the identity of the individual requesting disclosure, etc., by confirmation over the telephone. However, if telephone confirmation is not possible, you may be asked to submit a copy of your driver’s license, resident card, or health insurance card, etc.
4. Requesting disclosure, etc., by proxy
   When entrusting the request for disclosure, etc., to a proxy, please enclose the following documents with the personal information disclosure request form.
   1. Documentation confirming the proxy’s identity (copy)
      A copy of one of the following: driver’s license, resident card, national health insurance card. Please provide a copy with the permanent address blacked out.
   2. Letter of proxy (Please have the letter of proxy stamped by the individual, and append their seal registration certificate. If the proxy is a legal representative such as a person in parental authority, you may submit documentation of their relationship with the individual instead of a letter of proxy.)
5. Handling fee for disclosure requests, etc., and notification of purposes of use
   We require a handling fee of ¥500 (tax included) for each personal information disclosure request, etc., or notification of the purposes of use.
   Please pay the handling fee by depositing ¥500 (tax included) via bank transfer. During identity verification, we will give you the deposit account information. Please cover the cost of the deposit service fee. Additionally, please note that if the handling fee is not paid or the deposit cannot be confirmed, we cannot make disclosures or purpose of use notifications.
6. Method of response to disclosure requests, etc.
   We will respond via the method the requester used (email, fax, post).
