If the proxy is a legal representative such as a person in parental authority, you may submit documentation of their relationship with the individual instead of a letter of proxy.)
  5. Handling fee for disclosure requests, etc., and notification of purposes of use
    We require a handling fee of ¥500 (tax included) for each personal information disclosure request, etc., or notification of the purposes of use.
    Please pay the handling fee by depositing ¥500 (tax included) via bank transfer. During identity verification, we will give you the deposit account information. Please cover the cost of the deposit service fee. Additionally, please note that if the handling fee is not paid or the deposit cannot be confirmed, we cannot make disclosures or purpose of use notifications.
